Market vs Social Justice in Healthcare

Market justice and social justice are two contrasting approaches in addressing healthcare and other issues facing our society. Market justice derives from principles of individualism, self-interest, personal effort, and voluntary behavior, while social justice allocates goods and services according to the individual’s needs (Budetti, 2008). Market forces including ability to afford healthcare services based on individual’s efforts and financial resources drive the market justice system whereas principles of communal well-being, with government as the vehicle for ensuring equity is the driving force for social justice.

Situation

One situation I see as a tension between market justice and social justice system in healthcare in the US is the constant battle among politicians with differing ideology in terms of what to do with Affordable Health Care (ACA) once it was enacted as a law of the land. Many states chose to expand Medicaid under the ACA, thus allowing many citizens to access health insurance and increasing access to various healthcare services including preventative care, screenings, and other benefits (Affordable Care Act, n.d). Government was driving this with fundings ensuring that individuals who meet income criteria and have the needs get the Medicaid; this is social justice in action. States that didn’t expand Medicaid argue that they do not want government to be part of this, instead they would strengthen services under private insurance allowing market to drive the care; this is an example of Market Justice.

Nursing role in achieving social Justice:

Nurses can play critical role in identifying and addressing medical, social, and other issues facing our patients. Screening for social needs and making referrals to social services is becoming more commonplace in clinical settings as part of efforts to provide holistic care (National Academies Press, 2021). Through their assessment and appropriate referrals to community resources hence helping achieve social justice.

Market Justice– In such a system, medical care and benefits are distributed on the basis of people’s willingness and ability to pay (Rucci, 2023). An example of market justice is an employer-based health insurance benefit. In 
Social Justice, the equitable distribution of healthcare is a societal, rather than an individual responsibility (Rucci, 2023). Medicare and Medicaid are examples of social justice.

In a situation where I feel there is tension between the two, is an individual who is fully insured versus an individual who has no insurance. I have seen patients with insurance receive more diagnostic tests and their symptoms are further investigated than those without insurance. 

As nurses, we must continue to advocate for all patients regardless of financial status. We need to collaborate as a team with others in assessing the needs of less fortunate individuals and ensuring that they receive the same adequate care. In my previous research, it has been noted that the uninsured, low-income, and the homeless, often suffers more from conditions that can be easily managed with proper care and treatment. Social Justice is action-oriented. It needs to be applied in two levels: direct action to assure that the care of patients or populations is just; and action and advocacy at the level of policy and regulation to institute structures or to remove the barriers that keep people vulnerable (Donley, 2010).

What is QSEN?

The Quality and Safety Education for Nurses (QSEN) as described by Dolansky et al. (2013), is an initiative aimed at integrating quality and safety competencies into nursing education. It was developed in response to the need for improving the American healthcare system, as identified in the Institute of Medicine’s reports. It’s important because QSEN focuses on equipping nurses with knowledge and skills to enhance the quality and safety of healthcare delivery, with an emphasis on moving from individual patient care to a broader system thinking approach, ultimately aiming to accelerate improvements in healthcare quality and safety. 

Explain how QSEN relates to this course.

QSEN, the Quality and Safety Education for Nurses initiative, is closely related to research and evidence-based practice in nursing. It emphasizes the integration of research findings and evidence-based principles into nursing education, training nurses to make informed, evidence-based decisions to improve patient care and safety. This connection ensures we as nurses are exposed to research principles and critical thinking skills early in our education promoting a culture of quality and safety improvement in the healthcare system.

What is QSEN?

QSEN refers to nurse quality and safety education. It is a national initiative to improve patient care in the United States. In 2004, the National Quality Forum and the Institute of Medicine launched the initiative. The mission of QSEN is to improve patient care quality and safety by expanding nurse education and training in the United States. All nurses must be able to deliver high-quality, safe patient care that is focused on the needs of the patients they serve as part of the QSEN competency set.

 

Explain how QSEN relates to this course.

This course helps to develop a safety-first mentality that begins with inquiry and presence of mind, as well as a willingness to raise questions about practices that are possibly questionable. Whether that practice is based on the best evidence and is it open to constructive feedback.
