Scenario # 1

You are working in a community health clinic on a team that has an LPN, RN, and nurse practitioner. Your first client of the day is a tearful young woman who states that she has had a painful rash to her genital area for the past couple of days. She came to the clinic today because the rash is getting progressively worse. Her vital signs are: 100.5 (PO), 114, 28, 145/88 Pain: 9 on a 1-10 scale. The client shares with you that her boyfriend has herpes. They never have sex when he is having a flare-up, so she is not sure how she could have gotten it from him.

Question # 1: What should be the first priority for this client? Explain your answer. (5-10 sentences)

Question # 2: The RN provides teaching to the client, but the client has some follow-up questions. As the LPN you reinforce the teaching about how herpes is spread. What factual information should the nurse provide about the spread of this disease
between flare-ups? (2-3 sentences)

Scenario # 2

You are working in a primary care office with a physician’s assistant (PA) and RN. A client comes in for a follow-up visit. He states that he was called to come in because of an “abnormal lab report.” You print the labs for the PA, and you see that the client had a sore on his penis that was scraped and sent to the lab. The scrapings are positive for syphilis. You also see in the notes that the client had to be called repeatedly to come to the office. At first, he was unwilling to return because he stated he “feels fine.”

Question # 3: The client is confused about why he needs to name his sexual partners. What factual information can the nurse give to explain the need for all of his sexual partners to be treated as soon as possible? (5-10 sentences)

Question # 4: The PA orders a single dose of Benzathine Penicillin G 2.4 million units IM. The client questions why he just cannot take some pills. What explanation will you provide to the client? (5-10 sentences)

Scenario # 3

You are working in a primary care office with a physician. You are instructed to collect data on a client, including vital signs and chief complaint. The client states that she is a sex worker and has had chills, fevers, and sore throats on and off for several weeks. The physician orders a rapid HIV test.

Quality Improvement Presentation Poster

Create an abstract (100-250 words) and a poster for a presentation with voice-over for a quality improvement plan to executive-level leadership.

Introduction

Note: Each assessment in this course builds on the work you completed in the previous assessment. Therefore, it is recommended that you complete the assessments in this course in the order in which they are presented.

As a master’s-level nurse you will be responsible for promoting equitable quality of care. In the biopsychosocial model of care, there are a number of areas where you can have an impact on improving the quality, safety, and equitability of care. One example area where equitable quality of care is an issue is with medications, especially considering the cost of various medications to treat the same condition versus the effectiveness and potential side-effects of the medications. A further pharmacological concern for safe and quality care is medication errors. This could include considerations like a patient getting the wrong medication or a dangerous interaction due to other aspects of a patient’s health and treatment situation. An example could be vitamin or herbal supplements the patient has not disclosed as part of the medication reconciliation process.

The ability to propose and promote quality improvements in care is a vital skill for a master’s-prepared nurse. One aspect of this is understanding strategies for implementing best practice changes in a team, department, or care setting. Another is understanding how best practices can help lead to a continuous quality improvement cycle. Additionally, it is important to grasp how successful change strategies can be leveraged to help in the creation or implementation of clinical best practices. Part of this is considering the audience when proposing quality improvement projects or new best practices. Another part of this is being able to effectively communicate the goals, rationale, and benefits of a project.

One way to communicate the need for quality improvements, and your plans for achieving specific improvements, is through a presentation poster. This type of communication tool is used both in the work place and at professional and academic conferences. Being able to convey the essence and value of a project in a compelling and succinct way is a valuable skill, and it is vital within the constraints of a single poster.

As a master’s-level nurse, your specific focus is on reviewing the implications of the data relevant to the clinical issue you are trying to address. Once you research this, review the aggregate data, and understand the BPS considerations relevant to the clinical issue, you can suggest strategies for improving the quality, equitability, and safety of care around the issue. You should act as an advocate for the value and need to pursue quality improvements to leadership and executives, as well as be able to present potential projects to a wide range of colleagues and community stakeholders.

Create an abstract and a poster for a presentation with voice-over to your colleagues that will sell them on your quality improvement plan.

Abstract Requirements

· Your abstract should be 100–250 words.

· It should summarize the key information in your poster.

· Do not put your abstract on the poster itself; submit it as a separate document. Or, if you are using PowerPoint to help create your poster, create a new slide that is clearly labeled as your abstract.

Poster Requirements

· Your poster should include the following sections:

· Quality Improvement Methods.

· Evidence Supporting QI Methods.

· Change Strategy Foundation.

· Interprofessional Team Benefits.

· Overall Project Benefits.

· There are templates in PowerPoint or on the Internet that can help you get a start designing your poster.

1. You may build off your 2nd assessment for this assignment since you have already established a good foundation with change strategies and justification for it. This will be your quality improvement initiative or project.

2. You will need to add strategies to help implement your QI project because there is usually some resistance to change. There are many theories or strategies that can help assist with resistance to change, such as: PDCA or PDSA, Six Sigma, DMAIC, Lewin’s Change Management Model, McKinsey’s 7-S model, Kotter’s Theory, Nudge Theory, the ADKAR Model, Bridges’ Transition Model, Kubler-Ross’ Change Curve, and the Satir Change Model. You will need a strategy to help with the challenges of change and how to meet those challenges or the QI project may not be successful or may be halted. 

3. Remember to include potential complications or limitations for the QI project and strategies to assist with change. It is important to not just identify complications or limitations but also ways to overcome or lessen the negative impacts.

4. When putting the poster together, envision that you are presenting this to administrators, insurance payors, policy makers, and the community, not just to healthcare workers and patients/families. What can you add to appeal to all of these stakeholders to encourage buy-in? For example, will your QI project not only improve safety and equitable care, but could it reduce costs or expenditures? Will it make processes more efficient or more effective? Will it improve healthcare statistics for the community, state, or country? You want to get as many people on your side as you can.
